Our professional staff takes a comprehensive approach to developing an individualized plan of care for each participant. We assist in the management of the many challenging situations faced by persons living with HIV/AIDS. At the same time, we partner with the patients’ service providers to ensure seamless and individualized services.
Ultimately, the program provides a positive, community-based experience that is intended to improve a participant’s overall health.

We care for individuals with complex, extensive and challenging physical and social needs. Additionally, we offer participant centered programs focused on helping individuals living with HIV or AIDS, who may have behavioral health problems and other chronic illnesses lead healthier lives.
AIDS Care’s Day Health Program serves a light breakfast daily Monday through Friday and a hot lunch Monday through Thursday. Transportation assistance is provided.
Our services are available to Medicaideligible persons. Applicants for the Day Health Program must be at least 18 years old and require HIV/AIDS medical monitoring.
Will there be someone to help with care coordination at AIDS Care’s Day Health Program?
Individualized care and a participantcentered approach is the foundation of the program.
Every participant is assigned a case manager who will work with clients to develop an individualized plan of services to best meet their needs.
